29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) On or about June 09, 2015, the plumbed eyewash station near the sink in the Production Area was not adequate since one of the two nozzles was not dispensing water. b) There was not an eye wash readily available for employees dispensing corrosive cleaning production in the Boiler/Chemical Storage Area.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) The employer has not developed an energy control program for servicing and/or maintaining production and building equipment. All provisions of 29 CFR 1910.147 (c) through (f) must be covered in a lockout tagout program. Key elements include, but not limited to the following 1) Written lockout tagout program 2) Energy control procedures specific to each machine and equipment 3) Isolating devices, i.e. locks, tags, chains, wedges, key blocks, or other devices to isolate equipment energy sources 4) Periodic inspection to ensure procedures are being followed at least annually 5) Train and communicate program with authorized and affected employees 6) Application of control. preparation for shutdown, equipment shut down and verification 7) group lock out include for outside contractors
