29 CFR 1910.106(d)(4)(iv): Ventilation. Every inside storage room was provided with either a gravity or a mechanical exhaust ventilation system. If a mechanical exhaust system was used, it was controlled by a switch located outside of the door. The ventilating equipment and any lighting fixtures were not operated by the same switch. Where gravity ventilation was provided, the fresh air intake, as well as the exhaust outlet from the room, was not on the exterior of the building in which the room is located:    a)  On or about June 10, 2015, and at times prior, employees were exposed to fire hazards and chemical vapors because the mechanical exhaust for the paint storage room was turned off in the evening leaving only gravity ventilation.  The fresh air intake was not located on the exterior of the building and was instead located in an office area.  Vapors from the storage room could have migrated through the air intake and concentrated in the office creating chemical overexposures or flammable atmospheres.      b)  On or about June 10, 2015, and at times prior, employees were exposed to fire hazards and chemical vapors in the paint storage room because the ventilation equipment was not tied into the light switch making it possible to work in the room without adequate ventilation.

29 CFR 1910.106(e)(7)(i)(a): All electrical wiring and equipment was not installed according to the requirements of Subpart S of this part:  a)  On or about June 10, 2015, and at times prior, employees were exposed to fire hazards because the mechanical exhaust for the paint storage room was turned off in the evening leaving only gravity ventilation.  The fresh air intake was not located on the exterior of the building and was instead located in an office area.  Vapors from the storage room could have migrated through the fresh air intake and concentrated in the office creating flammable atmospheres.  Electrical wall/junction boxes within a foot of this intake were not marked or classified for a Class I, Division I flammable atmosphere.

29 CFR 1910.134(d)(3)(iii)(B)(2):  The employer did not implement a change schedule for respirators not equipped with an ESLI that would ensure cartridges were changed before the end of their service life:         a)  On or about June 10, 2015, and at times prior, the employer did not protect employees that used half-face air purifying respirators while spray-applying coatings containing isocyanates and other hazardous chemicals by developing and implementing a cartridge change schedule to prevent breakthrough of the organic vapor cartridges used with the respirators.
