29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection:      (a) Site- on or about June 10, 2015 -  An employee was exposed to electrical hazards while using a Dewalt Drilll that did not have GFCI protection.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access:      (a) Rear of the Building - on or about June 10, 2015 - an employee was installing stucco to the exterior of the building, working from a scaffold deck and did not have a ladder for access. Work elevation was approximately 16 feet, 10 inches from the ground.

29 CFR 1926.451(b)(1) : Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than one inch wide:    (a) Rear of the Building - on or about June 10, 2015 - an employee was installing stucco to the exterior of the building, working from a scaffold deck that was not adequately planked.  Work elevation was approximately 16 feet, 10 inches from the ground.      New York Stucco & General Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1), which was contained in OSHA inspection number 944713, citation number 01, item number 001 and was affirmed as a final order on 12/19/13, with respect to a workplace located at 393 Pacific Ave, Stamford, CT 06901.

Work elevation was approximately 16 feet, 10 inches from the ground.
