29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:     On or about June 9, 2015, at a job site located at 1572 SR 44, Randolph, Ohio 44265, during construction activities including but not limited to masonry installation, the employer did not establish and implement a written respiratory protection program. Employees were required to wear filtering face-piece respirators during cutting and mixing mortar while installing concrete block.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  [63 FR 33450, June 18, 1998]    On or about June 9, 2015, at a job site located at 1572 SR 44, Randolph, Ohio 44265, during construction activities including but not limited to masonry installation, the employer did not provide an eye wash station while using masonry materials that contained Portland cement.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    On or about June 9, 2015, during construction activities including, but not limited to, block laying work from an elevated platform, the employer did not ensure employees working on the elevated platform did not climb frame rails as a means of access to different levels.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system.      On or about June 9, 2015, during the installation of masonry materials on a jobsite located at 1572 SR 44, Randolph, Ohio 44265, the employer failed to protect its employees from a fall of approximately 19 feet by using a personal fall arrest system.
