Employees were required to wear filtering face-piece respirators during cutting and mixing mortar while installing concrete block.

[63 FR 33450, June 18, 1998] On or about June 9, 2015, at a job site located at 1572 SR 44, Randolph, Ohio 44265, during construction activities including but not limited to masonry installation, the employer did not provide an eye wash station while using masonry materials that contained Portland cement.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. On or about June 9, 2015, during construction activities including, but not limited to, block laying work from an elevated platform, the employer did not ensure employees working on the elevated platform did not climb frame rails as a means of access to different levels.
