29 CFR 1926.95(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:  Roof of hotel entrance driveway:  On or about June 19, 2015, an employee was exposed to the hazard of being struck by flying projectiles, in that the employee was using a pneumatic nail gun to attach asphalt felt roof paper without the use of safety glasses.

29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet or more above lower levels were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted:  Roof of hotel entrance driveway:  On or about June 19, 2015, employee(s) were exposed to a fall hazard greater than 6 feet to the ground level below, while attaching asphalt roof paper without the use of fall protection equipment or an alternative fall protection plan.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  Roof of hotel entrance driveway.  On or about June 19, 2015, employees were exposed to a fall hazard of up to 15 feet, as the ladder, used for access to the roof, did not have the side rails extending 3 feet above the landing platform.
