In the production area, polishing lathe, on or about 06/03/15: Rotating chuck on two polishing lathes were not guarded. Abatement certification must be submitted for this item. Hubbard Tool & Die Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 314350133, citation number 1, item number 4 and was affirmed as a final order on 06/01/11, with respect to a workplace located at 401 Mill Street, Building 5, Rome, New York, 13440.
