29 CFR 1926.602(c)(1)(ii): No modifications or additions which affect the capacity or safe operation of the equipment shall be made without the manufacturer's written approval. If such modifications or changes are made, the capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. In no case shall the original safety factor of the equipment be reduced. a. Located in Groveport OH near Medeira Dr and East Ridge Dr, the employer allowed employees to lift a reel of wire weighing approximately 1000 pounds with a wire rope sling attached to the tine of a Cat 950K wheel loader without prior approval from the material handler manufacturer. The use of a wire rope sling with a load below the tines can change the capacity and safe operation of the material handler.

Case number 15-003, JG on 3/19/15, did not include what caused the injury. 2. Case number 15-004, MF on 4/10/15, did not include what the actual injury to the lip was and what caused the injury. 3. Case number 15-005, SJ on 5/5/15, did not include what the actual injury was to the forearm. 4.
