29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury:  Well head area: On or about June 4, 2015, employees installing an oil/gas well's flow lines worked in a trench.  The employees had not been trained on excavation hazards and their prevention.  This exposed the employees to the hazard of being struck by falling soil.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:  Well head area: On or about June 4, 2015, employees installing an oil/gas well's flow lines worked in a trench.  The spoil bank on the north end of the trench sat on the edge of the trench walls.  This exposed the employees to the hazard of being struck by falling soil.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  Well area:  On or about June 4, 2015, and time prior to, employees installing an oil/gas well flow lines used electric handtools powered from a truck mounted generator through a cordset. The cordset was damaged and an electric handtool had a severely damaged attachment cord exposing the employees to the hazard of electrical shock.
