29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Workers were observed installing gutters and fascia of a residential style construction site (house) without wearing any form of fall protection.  The workers were exposed to falls of over 22 feet to grass and concrete below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   The employer is failing to provide employees with adequate and effective fall protection training prior to allowing them to work on a residential style roof.  This training would allow each employee to recognize the hazards of falling and allow them to understand the proper procedures to be followed in order to minimize these hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   The employer is failing to protect employees from fall hazards by not ensuring that ladders being used on site extend 3 feet above the upper working surface.  The employees are exposed to a fall from heights of greater than 9 feet to dirt and concrete below.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:   Employees had not been trained on the proper use of portable ladders.  Multiple hazards involving the set-up and usage of the ladder on site were identified during the inspection.  These hazards and the lack of training exposed the employees to a fall hazards in excess of 9 feet to the dirt and concrete on this site.
