29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about 11 June 2015, an employee was on the ground helping another employee from an aerial lift remove an overhead sign without the use of a hard hat, exposing the employee to the hazard of being struck by falling tools, debris, or the sign.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. On or about 11 June 2015, an employee was removing a sign from the side of a building while in the basket of an aerial lift without using a body belt or harness to tie off, exposing the employee to a fall hazard of approximately 16 feet to the concrete below.
