29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   (a) On or about June 11th, 2015 at worksite located at or near Ridgeway Dr. in Raymore, Missouri.-employees were exposed to serious eye injury while using a pnuematic nail gun without eye protection.    Abatement certification for this violation is not required.

29 CFR 1926.501(b)(1):   Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about June 11th, 2015 at worksite located at or near Ridgeway Dr. in Raymore, Missouri.-employees were exposed to a fall hazard when performing  framing  activities  at an elevation of approximately 40 feet  without an approved form of fall protection, to include, guardrail systems, safety net systems, or personal fall arrest systems.      Abatement Note: Abatement certification for this violation is not required.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On June 11, 2015 at the jobsite located at or near Ridgeway Dr. in Raymore MO employees were exposed to a fall hazard in that the ladder they were using did not extend three feet above the upper landing being accessed.
