29 CFR 1910.24(b): Fixed stairs were not provided for access from one structure level to another where operations necessitated regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations:  At this oil services site, employees were exposed to slip and fall hazards while working on piping equipment which was used as a platform access to operate equipment.  As a result, an employee slipped on this equipment  while performing routine operations.

29 CFR 1910.212(a)(5): Exposure of blades. When the periphery of the blades of a fan is less than seven (7) feet above the floor or working level, the blades shall be guarded. The guard shall have openings no larger than one-half (1/2) inch.   At this oil services site location, employees were exposed to a caught in between hazard which caused an amputation while working on missile piping equipment.   The motor of the equipment consisted of a panel covering with openings of more than 1 inch large and was not guarded.
