29 CFR 1910.212(a)(3)(ii): The employer did not ensure that a point of operation machine whose operations exposed employees to injury were guarded:       a) National Cart Co., Inc. @ 3125 Boschertown Rd., St. Charles, MO 63301: On and before June 11, 2015, the employer did not ensure that employees who operated the Amada hydraulic brake press (model number 3512LD, serial number 35120063) were protected against point of operation hazards. The gap between the punch and die was such that it allowed an employee to place his right pinky finger in the point of operation. The tip of the employee's right pinky finger was degloved as a result of the incident.

29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replaced:       a) National Cart Co., Inc. @ 3125 Boschertown Rd., St. Charles, MO 63301: On and before June 11, 2015 welders in the galvanized department MIG welded with Miller Deltaweld 302 welders that were rated at 300 amps at 32 volts direct current. Welding leads were damaged, in that the inner conductors were observed. This condition exposed employees to an electrical shock hazard.     b) National Cart Co., Inc. @ 3125 Boschertown Rd., St. Charles, MO 63301: On and before June 11, 2015, the employer did not replace damaged welding leads. The employer repaired the damaged welding leads by layering rubber splicing electrical tape, with electrical tape, and then friction tape. This condition exposed employees to electrical shock hazards.
