Coolsaet Construction Company on or about June 11, 2015, at the address of 4900 Laskey Rd. Sylvania, OH: The employer failed to ensure employees were adequately protected from cave-ins while performing upgrade work on natural gas piping at the base of a 7.5 ft. deep excavation.

Designs of support systems, shield systems, and other protective systems were not selected and constructed by the employer or his designed in accordance with the requirements of paragraph (c)(1); or, in the alternative, paragraph (c)(2); or, in the alternative, paragraph (c)(3); or, i the alternative, paragraph (c)(4) as follows: (a) R.L. Coolsaet Construction Company on or about June 11, 2015, at the address of 4900 Laskey Rd. Sylvania, OH: The employer failed to ensure that the employer designed/fabricated protective shoring utilized onsite was accompanied with engineered tabulated data, identifying the specifications and limitations of use. Without this engineered data, employees are unable to effectively assess the parameters in which the protective system may be safe to utilize.
