29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) On or about June 10, 2015, at 1223 Royal St., New Orleans, LA; an employee was attaching a strap to secure a gutter down pipe from an aerial lift bucket while not wearing fall protection. The employee was exposed to a fall hazard of approximately 14 feet above the ground.
