29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: A) Jonick & Co., Inc., located at 1840 Idaho Ave., Lorain, OH 44052: On or about June 1, 2015, the employer failed to establish a lockout/tagout program for the control of the hazardous energy, which includes procedures, training, and periodic inspections, while performing activities such as, but not limited to, changing screens on equipment such as, but not limited to, the McCloskey R155 Screener. Employees were exposed to hazards associated with moving parts while changing screens.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): A) Jonick & Co., Inc., located at 1840 Idaho Ave., Lorain, OH 44052: On or about June 11, 2015, the employer failed to provide training and evaluation to ensure that employees, operating powered industrial vehicles such as, but not limited to, the Komatsu WA500 loader (Model WA500-6, Serial # A92576) and the Komatsu WA480 loader, were competent to safely complete the task. Employees were exposed to struck-by, caught-in-between, and tip-over hazards.
