29 CFR 1910.132(b): Where employees provided their own protective equipment, the employer did not assure its adequacy, including proper maintenance, and sanitation of such equipment. (a) The employer did not inspect employee-owned equipment, such as respirators and prescription glasses, to ensure it was adequate as required for the welders who welded galvanized metal. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee who wore prescription lenses while engaged in operations that involved eye hazards wore eye protection that incorporated the prescription in its design, or wore eye protection that could be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses. (a) On or about June 15, 2015, a welding employee was observed wearing non-safety prescription lenses with side shields instead of safety glasses in the workplace. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user: (a) The employer did not implement the medical evaluation and training sections of the respiratory protection standard when they permitted welders who welded galvanized metal to wear respirators for voluntary use. 29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of zinc oxide fume listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 5.0 milligrams per cubic meter: (a) On or about July 1, 2015, a welder on the leg line was exposed to zinc oxide fume at 17.4 milligrams per cubic meter, or 348% of the OSHA permissible exposure limit, for a 452 minute sampling period with a value of zero added for the 28 minutes not sampled, while welding galvanized steel. (b) On or about July 1, 2015, a welder on the leg line was exposed to particulates not otherwise regulated (total dust) at 38.2 milligrams per cubic meter, or 255% of the OSHA permissible exposure limit, for a 452 minute sampling period with a value of zero added for the 28 minutes not sampled, while welding galvanized steel. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
