Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a.) AGSTEN Construction Company, Inc., 3601 James Street, Charleston, WV: On or about 6/12/15, employees performing roofing activities on a low slope roof were not protected from falling 20' to the ground. ABATEMENT DOCUMENTATION IS REQUIRED.
