29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: a) On or about June 11, 2015 - at job site at 30-30 47th Avenue, Long Island City, NY 11101 Employer allowed employees to work on top of a mobile scaffold on the interior side of a perimeter third floor window with three out of the four wheels in unlocked positions.
