OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, employees were exposed to fire and explosion hazards:       At the work site located at Roadrunner Well Service, LLC, Rig# 3, E 1520 Rd., Maysville, Ok: employees assigned to work from the Derrick Man's work platform at approximately 25 to 30 feet were exposed to fire and explosion hazards.  The employer did not provide an auxiliary escape line to the employees to be installed prior to employees performing work from the Derrick Man's work platform.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered.       At the work site located at Roadrunner Well Service, LLC, Rig# 3, E. 1520 Rd., Maysville, Ok: the employer did not ensure that an employee working from the Derrick Man's platform wore personal protection equipment while engaged in oil and gas drilling activities.  An employee did not have on Flame-Resistant Clothing (FRC) was exposed to fire hazards.
