29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, rotating and moving parts: a) Production/Specialties Area/Pettigrew Transfer Machine: On or about June 1, 2015, and times prior thereto, the rotating and moving vertical auger inside the materials elevator tube was not guarded at the bottom to prevent employees having fingers/hands near the auger during cleaning operations. b) Production/Specialties Area/Pettigrew Transfer Machine: On or about June 1, 2015, and times prior thereto, the rotating and moving horizontal auger inside the powder metal hopper was not guarded to prevent employees from having hands near the auger during cleaning operations.

29 CFR 1910.212(a)(3)(iii): Special handtools for placing and removing material were not such as to permit easy handling of material without the operator placing a hand in the danger zone: a) Production/Specialties Area/Pettigrew Transfer Machine: On or about June 1, 2015, and times prior thereto, during cleaning operations of the hopper, a special handtool was not used to prevent employees from having fingers/ hands near the vertical auger when removing powder from the materials elevator duct.
