29 CFR 1926.502(d)(1): Personal fall arrest system connectors were not drop forged, pressed or formed steel, or made of equivalent materials:   ((a) At the site the employee, painting the building, was exposed to a 17' fall hazard while connected to the AC unit on the roof by a rope. The connectors at the AC unit and the D Ring of the employee's harness were connected by knots tied in the rope, while working on a ladder scaffold on 6/11/2015.

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use:   (a) At the site the ladder used while painting was not inspected by a competent person for visual defects on a periodic basis. The ladder was determined to have defects such as, but not limited to: damaged safety feet with rusty  bolts exposing the employee to a 17 ft. fall hazard on or about 6/11/2015.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.   (a) At the site a ladder with structural defects was not removed from service, but instead was used while painting the west section of a hotel. The ladder had defects such as, but not limited to: rusty bolts on the safety feet exposing the employee to a 17 ft. fall hazard on or about 6/11/2015.
