29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about 6/15/2015, Horizontal conveyor in baler rom needed bottom guarding for ingoing nip points created by rollers and the belt. b) On or about 6/15/2015, Opening between conveyor and chopper needed guarding to prevent contact with chopper blades. c) On or about 6/15/2015, Clean out door for chute from chopper was being held in place with only one bolt of the six designed for it and that had a wing nut on it. d) On or about 6/15/2015, the incline conveyor had ingoing nip points that needed guarding in the baler room. e) On or about 6/15/2015, The baler had guards on top of it that were not secured or interlocked. f) On or about 6/15/2015, The front interlocked guard door on the Microliner did not shut down the die roll when it was opened. g) On or about 6/15/2015, The Microliner top guard was not interlocked. h) On or about 6/15/2015, The Microliner glue applicator ingoing nip point was not guarded. I) On or about 6/15/2015, The SFL machine had ingoing nip points that were not guarded for two coating and chrome rolls. j) On or about 6/15/2015, Procedures called for cleaning rolls with a rag and single jog button near ingoing nip point for SFL machine. k) On or about 6/15/2015, SFL machine rolls under lamination section contain ingoing nip points that need guarding. l) On or about 6/15/2015, SFL machine had guards open on the backside that no longer required special tools to be opened. m) On or about 6/15/2015, Envelope for robot had unguarded conveyors for moving racks in and out in the PSC machine department.
