29 CFR 1910.253(b)(2)(ii): Inside of buildings, cylinders were not stored in a well-protected, well-ventilated, dry location, at least 20 feet (6.1 m) from highly combustible materials such as oil or excelsior:  a. Near the bay door of the shop: On or about 06/10/2015, and at times prior, employees were exposed to fire hazards due to an acetylene storage cylinder being located within 20ft of oil storage containers.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written respiratory protection program necessary to ensure that any employee using a respirator voluntarily is medically able to use that respirator, and that the respirator is cleaned, stored, and maintained so that its use does not present a health hazard to the user.   a. On or before 6/10/2015, and at times prior, employees were exposed to hazards associated with not having a proper respiratory protection program in place.
