(a) 5050 Veterans Memorial Highway, Holbrook, NY (Worksite), Host Employer Facility - The employer failed to record the work-related injury, amputation, on the OSHA Form 300 for calendar year 2015 on, or about June 13, 2015. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this violation, in accordance with 29 CFR 1903.19.
