29 CFR 1910.176(a): Aisle(s) or passageway(s) were not kept in good repair:   a)    On the ramp connecting buildings 4 and 8: On or about June 4, 2015 and at times prior thereto, the employer exposed forklift operators to struck-by hazards, in that the ramp included an approximately 1 ft.-wide deteriorating hole, forcing operators to run over or navigate around it while traveling with loads up and down the passageway.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:   a)    On the ramp connecting buildings 4 and 8: On or about June 4, 2015 and at times prior thereto, the employer exposed an employee to serious injuries, in that a stand-up Raymond forklift operator was allowed to transport pallets without formal classroom-type instruction, and practical training not supervised by a qualified person.

29 CFR 1910.145(c)(2)(i): Caution signs were not used to warn against potential hazards or to caution against unsafe practices:  a)    At the LPG cylinder-storage rack outside building 4: On or about June 16, 2015 and at times prior thereto, the employer exposed forklift operators to fire hazards, in that the structure included no placards or signs of warning that flammable gases were contained when employees exchanged 33-lb. propane cylinders daily.
