29 CFR 1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use:  a) Sub cellar floor, south side:     Employees used an electric powered angle grinder to install floor tiles. The angle grinder was designed to accommodate a guard for the cutting wheel, and was not equipped with such guard when in use. On or about 06/10/15.     b) Cellar floor, north side:     An employee used an electric powered angle grinder to do framing work. The angle grinder was designed to accommodate a guard for the cutting wheel, and was not equipped with such guard when in use. On or about 06/10/15.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.     a) Sub cellar floor, south side:     Employees used an electric powered angle grinder to install floor tiles. The angle grinder was connected to an extension cord, which was routed to a nearby outlet device and was not provided with a ground fault circuit interrupter. On or about 06/10/15.     b) Cellar floor, north side:     Employees used an electric powered hand tools to do framing work. The tools were connected to extension cords, which were routed to a nearby outlet device and were not provided with a ground fault circuit interrupter. On or about 06/10/15.

29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.     a) Cellar floor, north side:     An energized electrical circuit breaker panel was not provided with proper cover. Its energized parts were covered with cardboard paper which does not provide insulation protection against accidental contact by employees. On or about 06/10/15.
