29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): On or about June 17, 2015, at the above addressed jobsite, a powered industrial truck (Daewoo SN:G6-00249) had a latch for the propane tank missing, exposing the employees to a fire hazard.

29 CFR 1904.40(a): The employer did not provide an authorized government representative (a representative of the Secretary of Labor conducting an inspection or investigation under the Act) the records within the four business hours: On or about August 27th,2015, the employer failed to provide copies of OSHA Forms 300 and 300-A or their equivalents, which were requested for calendar years 2010, 2011, 2012, 2013 and 2014 to date. The employer had several recordable injuries during the years requested.
