29 CFR 1910.178(m)(5)(iii): Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not lower the forks to the traveling surface and disengage the engine. (a) Yard, Sample Area: On or about June 16, 2015, an employee operating a powered industrial truck dismounted the running forklift while the load was lifted well above the traveling position, exposing employees to a struck-by hazard.
