29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a: Employees were exposed to a fall of approximately 10 feet to the concrete below, while working on a low slope roof, where fall protection had not been used or provided.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support)  Employees were using a Werner aluminum extension ladder that was set up with a working length of approximately 13 feet and the distance along the ladder between the foot and the top support was approximately 5 feet 5 inches resulting in a greater than a 4 to 1 ratio exposing the employees to fall and struck-by hazards.
