29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    The employer does not guard all the rotating parts of machines to protect the operator and other employees in the machine area.       a. This violation occurred on or about June 8, 2015, and at times prior thereto, in the Lathe Center where employees were exposed to struck-by and caught-by hazards while hand polishing parts being rotated in a Mori-Seiki brand CNC metal lathe without a functioning interlocking door guard.         b. This violation occurred on or about June 12, 2015, in the Horizontal Mill Center where employees were exposed to struck-by and caught-by hazards when operating Toyoda brand CNC metal mills without using interlocking door guards.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all machines that have exposed rotating parts are guarded.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.    The employer does not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. This violation occurred on or about June 12, 2015, and at times prior thereto, throughout the facility where employees were exposed to caught-in, struck-by, and electrical hazards while performing maintenance and servicing work on machinery without clear and specific steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.       Axon Energy Products was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(ii)(B), which was contained in OSHA inspection number 1030526, citation number 1 item number 1a and was affirmed as a final order on May 19, 2015, with a final abatement date of May 30, 2015, with respect to a workplace located at 80 N FM 3083 West, Conroe, TX, 77303.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that energy control procedures clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  The employer does not ensure that all energy isolating devices that were needed to control the energy to machines or equipment are physically located and operated in such a manner as to isolate the machine or equipment from the energy sources.    a. This violation occurred on or about June 12, 2015, and at times prior thereto, in the boring mill area of the facility where an employee was exposed to struck-by and caught-by hazards while bypassing a safety guard to setup a Toshiba brand CNC horizontal boring mill identified by the employer as C11 without ensuring that energy isolating devices were located and operated to isolate energy sources.    b. This violation occurred on or about June 12, 2015, and at times prior thereto, in the boring mill area of the facility where an employee was exposed to struck-by and caught-by hazards while bypassing a safety guard to setup a Toshiba brand CNC horizontal boring mill identified by the employer as C12 without ensuring that energy isolating devices were located and operated to isolate energy sources.    c. This violation occurred on or about June 8, 2015, and at times prior thereto, in the boring mill area of the facility where an employee was exposed to struck-by and caught-by hazards while bypassing a safety guard to setup a Toshiba brand CNC horizontal boring mill identified by the employer as C13 without ensuring that energy isolating devices were located and operated to isolate energy sources.
