29 CFR 1926.404(f)(6): Grounding path. The employer failed to keep a path to ground from circuits, equipment, and enclosures as permanent and continuous. On or about June 16, 2015, at job site location 1111 Tulane Ave, New Orleans LA 70112, the employer allowed the employee to use electrical equipment with the electrical plug missing a ground pin. Therefore there was not a permanent and continuous ground.
