29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) 3001 Archer Drive, Chambersburg PA; T300 Belt Conveyor - The employer does not ensure that employees utilize procedures developed and documented for the control of hazardous energy where employees perform preventative maintenance activities. This violation occurred on or about June 12, 2015 when a maintenance mechanic was injured when the air gun being used to blow down the conveyor belt and associated parts became caught in the running belt. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) 3001 Archer Drive, Chambersburg PA; T300 Belt Conveyor - The employer failed to shut down or turn off the machine to perform servicing in accordance with their established procedure, thereby exposing employees to the hazards of moving machine parts and in-going nip points. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], stored energy [(d)(5), if applicable], and verification of isolation [(d)(6)], were not implemented to safeguard employees from the machine servicing hazards. This violation occurred on or about June 12, 2015 when a maintenance mechanic was injured when the air gun being used to blow down the conveyor belt and associated parts became caught in the running belt.
