29 CFR 1910.136(a): The employer did not ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet are exposed to electrical hazards:  The employer did not ensure employee's feet are protected with safety shoes while they are working in area/s where large metal cylinders are being handled/worked on. Employees are exposed to a struck-by hazard.

29 CFR 1910.184(e)(1): Alloy steel chain slings shall have permanently affixed durable identification stating size, grade, rated capacity, and reach.  Alloy steel chain slings did not have permanently affixed identification as required in that the Serial numbers, reach, grade and Size were missing and/or no longer legible. Employees were exposed to the hazard of being struck by a dropped/uncontrolled load.

29 CFR 1910.178(l)(6): At this establishment, the employer did not certify that each operator has been trained and evaluated to operate the powered industrial truck as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  The employer did not ensure that certification of training was maintained for the operators of the forklift/s utilized on site.
