29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. On or about 25 May, 2015 an employee was exposed to an unguarded point of operation on a stave saw, the employee had a partial amputation as a result of contact with gears on the saw.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. On or about 25 May, 2015 the employer failed to report, within twenty-four hours, a work related incident that resulted in an amputation.
