29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer failed to guard the crimper machine Schleuniger CT 32 to prevent accidental contact with the point of operation. Employees were exposed to lacerations, fractures and amputations. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On June 07, 2015 an employee was injured while operating a crimper machine Schleuniger CT 32. The employees right index finger was caught in the machines point of operation. The resulting injury was a partial amputation of the finger. Abatement documentation is not required for this item.
