Employees, while operating the car wash, were exposed to the following hazardous chemicals such as but not limited to:  AC-3 Foam cleaner which contained phosphoric acid, sulfonic acids, and hydrofluoric acid; ShorePrep Gun Soap which contained potassium hydroxide;  Hi Shine Rinse-Aid which contained glycolic acid; CSD-50 which contained acetic acid, sulfonic acid, and glycolic acid; Wheel Brite which contained hydrochloric acid, ammonium bifluoride, and hydrofluoric acid; Tire Magic 925 which contained 2-butoxyethanol, potassium hydroxide, and alcohols; Tire Shine which contained alcohols ethoxylated;  and Flash Dry Plus which contained 2-butoxyethanol and acetic acid.

29 CFR 1910.1200(h)(1): The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees had not previously been trained about was introduced into their work area:   a) Worksite, On or about June 17, 2015 -  The employer did not provide effective information and training on hazardous chemical used throughout the car wash. 29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition:    a) Worksite; Front of car wash, On or about June 17, 2015 - The hose used to apply pre-wash soaking solution was tangled and wrapped on the ground.  This condition caused a tripping hazard.    b) Worksite; Front of car wash, On or about June 17, 2015 - The metal grate flooring was loose in addition it had large spaces in between the metal and broken concrete causing tripping hazard.

The area was blocked by movable carts stocked with paint, supplies and automotive tire cover.

29 CFR 1910.1200(f)(9): The employer defaced existing labels on incoming containers of hazardous chemicals, and was not immediately marked with the required information:  a) Workplace; Door near the entrance of the car wash, On or about June 17, 2015 -  A bucket of CD-50 which contained acetic acid, sulfonic acid, and glycolic acid was not labeled with a clear marking and was worn.    Employees used this hazardous solvent on each car to clean the back end of the vehicle.
