29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. Multiple employees were observed working on the roofs using pneumatic nail gun less than 2 foot away from their face without using eye or face protection, exposing employee to struck by hazards from flying debris or nail.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. Employees were observed performing roofing construction on both sites without adequate fall protection. This condition exposed employee to the hazard of falls from an elevation greater than 10 feet.
