29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: (a) Workplace - Appendix D information had not been provided to the custodian voluntarily permitted to use a 3M 8511 Model filtering face-piece respirator while conducting housekeeping operations, on or about 6/16/2015. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1919.1200(h)(1): Employees were not provided information and training as specified in 29 CFR 1910.1200(h)(2) and (3) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area: a) Workplace - Training had not been provided for a custodian exposed to various hazardous chemical cleaning products including but not limited to Clorox Bleach, used when performing cleaning operations, on or about June 16, 2015. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
