29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet (1.22 m) or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:    A). On or about May 29, 2015 at Bunzl Distribution Inc. at 1915 East Florida Street, Springfield MO: Employees were exposed to fall hazards 6 - 12 feet from lower level, by not requiring employees to wear fall harnesses while working from heights on mobile platform industrial truck pickers. Causing one employee to fall and receive head injuries, broke both wrists requiring surgery to repair them.

Employees on working platforms must be protected by a personal fall arrest system meeting the requirements of appendix C, Section I, of this standard, and as otherwise provided by this standard.     A). On or about May 29, 2015 at Bunzl Distribution Inc. at 1915 East Florida Street, Springfield MO:  Employees were exposed to fall hazards 6 - 12 feet from lower level, by not requiring employees to visually inspect fall harnesses before each use of working from heights on mobile platform industrial truck pickers. Causing one employee to fall and receive head, neck, eye injuries and broke both wrists requiring surgery to repair them.

The certification must include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.    A). on or about 29 May, 2015 at Bunzl Distribution Inc. at 1915 East Florida Street, Springfield MO: Employees were exposed to struck against, caught in, fall from and other hazards associated with power industrial trucks. By not certifying that all employees had up to date operator training certification.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.     A). On or about May 26, 2015 employer Bunzl Distributions Inc. 1915 East Florida Street, Springfield MO: The employer failed to report a hospitalization of an employee to Occupational Safety and Health Administration (OSHA) with in 24 hours of the employee being admitted into the hospital.  An employee was admitted to the hospital on or about 26 May 2015 at about 2:30 pm, and was not reported until 28 May 2015 at about 8:15 AM.
