In the basement of the house where an employee was using an extension cord, the ground prong was missing, thereby exposing the employee to an 125 volt electrical contact hazard.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. a. In the basement of the house where an employee was using an extension cord, the strain reliefs of both plugs were pulled away from the outer insulation jacket of the cord, thereby exposing the employee to a 125 volt electrical contact hazard.
