Prentice Drive, Littleton, CO 80022: On and before June 17th, 2015, the exposing employer did not ensure that adequate protective systems were provided for employees erecting concrete forms in an excavation. The condition exposed employees erecting forms to entrapment and cave in hazards while working in an excavation approximately 7 feet deep and between the forms and the excavations vertical wall that measured approximately 14 inches in width.
