This violation occurred on or about June 18, 2015, in Pod E where employees were exposed to struck-by hazards while operating a Toyota Forklift, SN 97141 that the parking brake did not work when fully engaged. c. This violation occurred on or about June 18, 2015, in Pod A where employees were exposed to caught in or between hazards while operating a Toyota Forklift, SN 76186 that had a broken gauge cover and would not shift forward or backwards due to defective hydraulics.
