29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: On or about June 13, 2015, employees were excavating for storm drain installation when the excavator bucket struck a gas line, exposing employees to the hazards of fire and/ or being struck by debris in the event of ignition of gas and/or explosion.
