At this jobsite, located off Milwaukee, the employer failed to ensure that a CMS metallic platform attached to the forks of a Rough Terrain Powered Industrial Truck was designed and approved to lift employees onto the roof of the building.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about June 19, 2015, at this jobsite, the employer failed to ensure that employees working on the roof of the structure more that six feet above the next lowest level had an approved method of fall protection/prevention, exposing employees to fall hazards.
