29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraph's (i) - (iii) of this paragraph. On June 19, 2014 a supported scaffold with a height to base ratio of 5:1 was not restrained by guys or ties, exposing employees to the hazards associated with a scaffold collapse. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
