29 CFR 1910.147(c)(7)(i)(A): Each authorized employee did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a) Laundry Department - On or about June 16, 2015, the employer did not ensure employees engaging in servicing and maintenance activities, such as aligning the apron on the Super Sylon Flatwork Ironer #2, Serial #128M3134, received training at the authorized level that included the types and magnitude of the energy sources in the workplace, and the methods and means necessary for energy isolation and control.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a) Laundry Department - On or about June 16, 2015, the employer did not ensure the following steps for de-energization during servicing and maintenance of the Super Sylon Flatwork Ironer #2, Serial #128M3134, were achieved:   (1) Turn off and shut down power to the machinery, (2) Physically locate and operate energy-isolating devices in such a manner as to isolate the machine or equipment from the energy source(s), (3) Affix lockout/tagout devices to each energy isolating device, (4) Verify isolation and de-energization of machinery have been accomplished prior to starting the maintenance and/or servicing work.  Employees were exposed to entanglement and amputation hazards from unexpected start-up and energizing of the Super Sylon Flatwork Ironer machinery.
