29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  a.  On or about June 19, 2015, located in Building #3 at #1 and #2 Eddy Current, employees were required to utilize an air nozzle within inches where the material came in contact with rotating wheels and guide rollers, thereby exposing the employees to ingoing nip point hazards.

29 CFR 1910.333(a)(1): "Deenergized parts." Live parts to which an employee may be exposed shall be deenergized before the employee works on or near them, unless the employer can demonstrate that deenergizing introduces additional or increased hazards or is infeasible due to equipment design or operational limitations. Live parts that operate at less than 50 volts to ground need not be deenergized if there will be no increased exposure to electrical burns or to explosion due to electric arcs.    a.  On or about June 13, 2015, located at the Extrusion Press Control Panel, employees were troubleshooting an equipment malfunction that had occurred with the Extrusion Press.  The electrical power to the control panel had not been deenergized prior to work, thereby exposing the employees to an electrical shock hazard of 110 VAC.
