29 CFR 1910.178(n)(6): The driver was not required to look in the direction of, and keep a clear view of the path of travel. 525 Mt Tom Rd, Northampton, MA On 6/16/15 and at times prior, a powered industrial truck operator frequently drove with view obstructed by a roll clamp attachment. On 6/16/15, the operator struck a pedestrian employee while operating the PIT with the roll clamp attachment obstructing the view.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: 525 Mt Tom Rd, Northampton, MA On 6/26/15 and at times prior, employees who operated powered industrial trucks did not receive triennial performance evaluations. Operators only received classroom training and a written exam. Packaging Corporation of America was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 943300, citation number 1, item number 1 and was affirmed as a final order, with respect to a workplace located at 3200 Hipack Dr., Opelika, AL.
