The employees were not protected with fall protection systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS) a) On or about June 17, 2015 and at times prior, employees were exposed to 30 feet fall hazards while using an extended ladder to access and egress the roof of a residential house. The rungs of the ladder was not extended at least 3 feet high above the landing surface.
